Sliding Scale Discount Fee:
We do not refuse services to anyone based upon their ability to pay. For that reason, we offer a sliding scale discount service program. When you call to set up your appointment, please let our office staff know that you would like to apply for the sliding scale discount program.
The sliding scale discount can be applied to both cash pay and insurance pay services. If you have insurance, the sliding scale may reduce your co-pay. Our nominal fee is $20.
A Mindful Life Counseling Services will determine program eligibility on a person’s ability to pay and will not discriminate on the basis of age, gender, race, sexual orientation, creed, religion, or national origin. Federal Poverty Guidelines are used in developing and updating the Sliding Fee Schedule (SFS) to determine eligibility.
Income Verification
Applicants are required to provide income verification as part of the application process.
Accepted forms of income verification should be one of the following: last 2 paycheck stubs for each household adult, signed statement from employer on letterhead, unemployment benefits letter, Social Security benefits letter, last (current) income tax return, or supported income statement (link below).
Supported Income Statement
If you do not have income and are supported by someone else, please have that person fill out the Supported Income Statement and provide that as income eligibility.
Once you have completed your review of the guidelines and can provide the necessary income verification, we will contact you to let you know about the status of your application and fee assignment.
